1. Register for USAV Membership
Screenshot the confirmation email with players registration number (you will need this for tryout registration!), or download the digital card on your smartphone. *Note: Digital cards can only be accessed from a smartphone.
2. Complete & Notarize the Medical Release Form
Print and NOTARIZE medical release form and bring to tryouts. We cannot guarantee we will have a Notary at our tryouts, please try and get this document notarized ahead of time!
3. Register For Tryouts
Note: You will be required to confirm your USAV membership number in the form.

Athletes are expected to attend both tryout dates!

11U Tryouts will be held on Sunday October 19th from 12:30PM-3:30PM & Monday October 20th from 5:00-7:00PM at the University of West Florida HLS Building. Address is : 11000 University Pkwy Building 72, Pensacola, FL 32514.

Teams will be selected within a few days after tryouts. Once teams are set, coaches will be contacting the players to schedule a team meeting to go over the tournament schedule and finalize practice dates.
